Physical Examination form is attached for your convenience. Open, print, complete the family history portion, and bring the remainder to your physician for completion at the time of exam. This form is valid for the 2008-2009 school year. Please note that a physician's office form may be used, however, a signed consent statement, as is now included on the MSHS physical form, MUST be included for a student's physical to be valid beginning with the 2008-2009 school year.

Parents are encouraged to complete the Program Assessment Form for Parents following each season that their son(s)/daughter(s) participate in order to provide feedback to the Athletic Department.

Students are encouraged to complete the Program Assessment Form following each season of sport participation in order to give the Athletic Department feedback.
